Instructions

1. In Row 3 next to 'HQ', enter your complete information for your NPA's headquarters.
2. Include the full location information for each additional project location or contract site for your NPA in the rows titled 'Project Location'. We understand that some NPAs will have no additional project locations while others will have 30+. Each contract site should be have it's own row.
3. Please return the completed spreadsheet to Advocacy@SourceAmerica.org
